DHN Provides 4 Levels of Participation

Partner

DHN provides guidance and support in the development, implementation and use of advanced HIT. Today’s healthcare environment demands that providers actively reach their patients to proactively manage their health conditions, follow-up with recommended care and stay healthy. Advanced use of HIT makes it easier to accomplish that critical patient care support.

Partner members are DHN Federally Qualified Health Centers (FQHC) and eligible as DHN Board of Directors.

HCCN Participants

Participating members are DHN Federally Qualified Health Centers (FQHC) that are eligible for Partnership following a 3-year member commitment.

Development

Patron members are community-based primary or behavioral health care organizations that are Ex Officio members of the DHN Board of Directors.

Benefactors

Benefactor members are health care providers or organizations that share DHN’s mission to improve community health outcomes and support DHN members in achieving this goal throughout Delaware.
