DHN is a (501c3) nonprofit organization that is pioneering a more coordinated approach to patient care through advances in data management and enhancements in administrative, clinical and financial information systems. Our Results

DHN knows how to optimize health information technology. FQHC and Primary Care are our passion. Delaware Health Network participants are health centers who collaborate as “safety net providers” to improve access to care, enhance quality of care, and achieve cost efficiencies through the redesign of practices to integrate services, optimize patient outcomes and enable its members to focus on what they do best — provide direct clinical health services to the most underserved communities and populations.

DHN is a HRSA funded “Health Center Controlled Network” HCCN, through the Bureau of Primary Health Care, to support health centers in full optimization of their EHR/Health IT systems.

Our Mission

Delaware Health Net, Inc is a 501c(3) non-profit, HRSA-funded Health Center Controlled Network (HCCN) that supports community health centers and other safety net providers optimize health information technology. Delaware Health Net’s unique, experiential training and technical assistance approach is tailored to meet the individualized, evolving needs of participating health centers to improve clinical quality and operational efficiency. DHN’s subject matter experts support practices with data driven processes to optimize their delivery of quality, patient-centered health services to the most underserved communities and populations.
