Nemours Health Clinic Pharmacy Assistance www.nemours.org/no/ nhc/svcs/pharmceutical.html

Delaware residents 65+ with yearly incomes of $12,500 (single) or $17,125 (couple) or less.  Must make an in-person appointment to apply.  Discounts on Rx on formulary.  20% co-pay per Rx.  $5.00 service fee.  Rx may be picked up at the main clinic by the patient or a representative with patient's ID card. Rx may also be delivered to one of several other locations. Call for more information.

DE Delaware Prescription Assistance Program (DPAP)

800-996-9969, press option 2 or request ext. 17 or 302-577-4900 Delaware residents 65+ or receiving Social Security Disability Insurance (SSDI) with yearly income of $17,724 (single) or $35,448 (couple) or less.  Individuals with incomes over these amounts may also be eligible if Rx costs are over 40% of yearly income.  Must not be eligible for Medicaid or have other insurance that provides drug coverage.  Must not be eligible for the Nemours program. Discounts depend on Rx.  $5 or 25% co-pay per Rx, whichever is greater.  Maximum annual benefit of $2,500 per person.
